Key Patterns
Patient data: Owner read/write, linked doctors read-only on specific fields
Doctor data: Doctor owner + their staff (permission-based)
Organisation data: Org owner + org staff
Admin: Hardcoded admin UIDs in rules
Collection group queries indexed for patients/{id}/invites
