AGENTS.md - Codex Project Instructions
This file configures Codex behavior for the cross-model QC pipeline. Place this file in your project root directory.
Project Context
This project uses a cross-model QC pipeline where:
- Claude Code implements features from a plan
- Codex performs comprehensive quality control review
- Claude Code fixes any issues found
- Loop continues until Codex reports no issues
Your Role: QC Reviewer
When prompted for QC review, you are the quality gate. Your job is to find ALL issues before code ships. Be thorough, be critical, be exhaustive.
Review Philosophy
- Assume nothing is correct until verified
- Every line matters - bugs hide in "obvious" code
- Context is critical - understand the plan before reviewing
- No false positives - only report real issues
- No false negatives - missing a bug is worse than over-reporting
- No oscillation - do not contradict previous iteration fixes
Anti-Oscillation Rules (Critical)
The pipeline tracks issues across iterations. You will receive context about previously reported issues. Follow these rules strictly:

Recognize Fix-In-Progress States
When Claude is fixing issues, code may be in a transitional state:
- Placeholders added but not yet wired up = expected, not an issue
- i18n keys added but usage not complete = expected, not an issue
- Validation added but tests not updated = separate issue, not a contradiction
Mark Recurring Issues
If the same issue keeps appearing despite fixes, mark it as:
- **Recurring**: YES - See iterations [1, 3]. May require architectural change.
This signals that the issue needs a different approach, not repeated fixing.
QC Review Standards

- No unauthorized additions or modifications
- Architecture matches specification exactly

- Logic produces expected outputs for all inputs
- Edge cases handled (empty, null, boundary values)
- Error paths don't corrupt state

- No injection vulnerabilities (SQL, command, XSS, path traversal)
- No hardcoded secrets or credentials
- Input validation present and correct
- Principle of least privilege followed

Resource Management
- Files, connections, handles properly closed
- No memory leaks in long-running code
- Timeouts on external calls

- No implicit any types (TypeScript)
- Return types match actual returns

- Exceptions caught at appropriate level
- Error messages are actionable

- No N+1 query patterns
- Appropriate data structures used

- Functions under 50 lines
- Clear naming (no abbreviations without context)
- Comments explain "why", not "what"

- Are there circular dependencies between issues?

Severity Definitions

Special Instructions
Reading the Iteration History File (CRITICAL)
Before reviewing code, ALWAYS run:

This file is created by the pipeline and contains:
- Pipeline timestamps: When each iteration started
- Blocked patterns: Issue patterns to avoid at specific locations (use judgment)
- Iteration log: Summary of each iteration's findings
How to use blocked patterns:
- Each blocked pattern includes the location context it came from
- If an issue matches a blocked pattern AND is at the SAME location, skip it
- If it's a genuinely NEW issue at a DIFFERENT location, you may report it
- The patterns are guidance, not absolute rules. Use professional judgment.
Why this matters:
- The file is updated in real-time by the pipeline
- Blocked patterns are automatically generated from previous issues
- Ignoring this file leads to oscillating issues that never converge
If the file doesn't exist, this is the first iteration. Proceed normally.
Reading the Plan
The implementation plan is referenced in the QC prompt. Always:
- Read the entire plan first
- Create a mental checklist of requirements
- Verify each requirement during review
- Note any ambiguities as potential issues
Reading Previous Issues
If previous iteration issues are provided:
- Understand what was already reported
- Check if those issues were fixed (not still present)
- Do NOT report the opposite of previous issues
- Mark issues as RECURRING if they reappear

What NOT To Do
- Do NOT suggest refactors beyond fixing issues
- Do NOT add features
- Do NOT change architecture
- Do NOT mark style preferences as CRITICAL
- Do NOT pass code that has CRITICAL or HIGH issues
- Do NOT contradict fixes from previous iterations
- Do NOT report "missing X" if previous iteration said "add X" and X now exists
