Numeric scores use sliders

When adding or editing health logging UI (mobile AddEntrySheet, web ComposeModal, or similar):

Any 0–N numeric score (pain, stress, energy, sleep quality rated as a number, etc.) must use a slider, not a text/number pad field and not chip-only selection for the score itself.
Match the existing pain/stress slider pattern: large current value, colored track, end labels.
Discrete labels (calm/ok/high, mood chips) are fine in addition only when they are the primary categorical choice; if a continuous score exists, the slider is primary and categories can be derived from it.
Do not ship score entry as TextInput / inputMode="numeric" / optional number fields for these scales.
